Student Core Didactic IPE Session: Root Cause Analysis & Error Disclosure Workshop

Emphasizing Values & Ethics for Interprofessional Collaborative Practice

In this core IPE session, students explore the ethical considerations surrounding a medical error, recognizing the principles of communication, team-collaboration, empathy, and emotional intelligence. Tailored for second year/advanced students, this session provides an opportunity for students to learn the complexities of healthcare systems and how different factors are interconnected and can lead to a cascade effect, resulting in a medical error.  

During the session, students will engage in a collaborative, virtual activity focused on learning and applying the fishbone diagram technique for conducting a root cause analysis of medical errors. Following this, students will utilize a provided template to develop an interprofessional communication plan and practice the sensitive task of disclosing the medical error to a simulated patient, family member, or caregiver. Emphasis is placed on maintaining a patient-centered approach during the error disclosure, with students learning to prioritize the well-being and needs of the patient and family involved.  

This session is strategically designed to enrich the student experience by applying the core Interprofessional Education Collaborative’s (IPEC) competencies. These include communication, teams & teamwork, and roles & responsibilities, with a specific focus on values & ethics. Through these immersive activities, we aim to not only deepen students’ understanding of their respective roles within healthcare teams, but also foster a collaborative mindset that aligns with the core principles of interprofessional practice.  

Estimated Time Commitment

  • 1.5 hours session preparation/pre-work  
  • 2.0 hours live virtual didactic session  
  • 0.5 hours post-session survey & reflection
