Title IX Report Form

The Title IX Coordinator is available to provide you with answers to question you may have about the process and your options and/referrals.  You are not obligated to use your name, however, failure to allow us to use your name may limit action that PNWU can take in response to your concerns or allegations. If the information you provide leads to a reasonable belief Sexual Harassment has occurred, the university may be obligated to investigate and take appropriate action.

Complaints must be filed as soon as possible or within 30 days of the event unless there are extraordinary circumstances that prohibit the complainant from reporting the alleged discrimination or harassment.  The Title IX Coordinator or Human Resources is available to assist you in stopping the unwelcomed and unwanted conduct and/or behaviors.
